FINAL FORMS for Parents/Athletes:
PLEASE DO NOT START THE 24-25 YEAR UNTIL AFTER JUNE 10, 2024. THAT IS WHEN THE NEW YEAR WILL ROLLOVER. IF YOU COMPLETE THIS BEFORE THEN, YOU MAY HAVE TO DO IT AGAIN!
GO to https://badin-oh.finalforms.com to begin. Returning students/parents need to update all forms in red. Remember students need to sign some forms as well and can only use their Badin email address in Final Forms.
FRESHMAN AND NEW STUDENTS will not have access to their Badin email until after June 15th, 2024. Here are the instructions for NEW ACCOUNTS:
Important: Parent/Guardian & Student accounts are saved year-to-year. If you already have an account with Final Forms please click here to login or reset your password.
- Register as a Parent/Guardian below.
You must be a parent/guardian to create this account and start the process. - Check your email (including your SPAM folder).
- You will receive an email within 3 minutes prompting you to confirm and complete your registration.
- You will then find yourself logged in and able to register your students!
If the student is playing ANY sport you will need to pick that sport on Final Forms even if you have not tried out yet.
ALL athletes MUST have a current physical turned in before try-outs. Physicals can be printed directly from Final Forms – and they are filled out! Please turn this in to Sharon Adams in the athletic office. You can email a copy to sadams@badinhs.org or upload a copy to Final Forms.
ALL forms must be completed and signed by a parent and student. Please make sure you have picked a sport before completing this. Students can ONLY use their Badin email in Final Forms.
If a student does not have a current physical or a parent/student has not signed their forms in Final Forms, that student will be marked as eligible and will not be able to try-out for his/her sport.
When all forms are complete, everything in the left column will be green. When this is done you have completed your account in Final Forms!
